Adult Recreation Liability Waiver

Liability Waiver and Release

By participating in any adult recreation activities offered by the Town of Thomaston and the Thomaston Recreation Department, including but not limited to basketball, softball, and kickball, I acknowledge and agree to the following:

1. Assumption of Risk I understand that participation in recreational sports and activities involves inherent risks. These risks include, but are not limited to, falls, collisions, equipment-related injuries, weather-related hazards, and other unforeseen dangers. I voluntarily choose to participate and accept full responsibility for any injury, illness, or harm that may occur.

2. Release of Liability I hereby release, waive, and discharge the Town of Thomaston, the Thomaston Recreation Department, its employees, volunteers, officials, and representatives from any and all liability for personal injury, illness, disability, death, or loss or damage to personal property arising from or connected to my participation in these activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Personal Belongings I acknowledge that the Town of Thomaston and the Thomaston Recreation Department are not responsible for any lost, stolen, or damaged personal items.

4. Medical Treatment In the event of an emergency, I authorize the Thomaston Recreation Department and its representatives to seek medical care on my behalf. I understand that I am responsible for any associated costs.

5. Compliance with Rules I agree to follow all program rules, facility guidelines, and instructions from staff or volunteers while participating in recreation activities.

6. Photo and Media Release I grant permission to the Town of Thomaston and the Thomaston Recreation Department to photograph, record, or otherwise capture my image or likeness during participation in recreation activities for lawful purposes including promotional materials, social media, publications, and public information. I understand that no compensation will be provided for such use, and all media becomes the property of the Town of Thomaston. If I do not wish to be photographed or recorded, it is my responsibility to notify staff in writing prior to participation.
